We are proud to offer the Enhanced Health in Care Homes (EHCH) Service, a comprehensive approach designed to improve health, wellbeing, and quality of life for all residents. This leaflet provides an overview of what the service is, what you can expect, and how it benefits you and your family.

🌟 What Is the EHCH Service?

The Enhanced Health in Care Homes Service is a national initiative that brings together health and social care professionals to deliver proactive, personalised, and coordinated healthcare support to people living in care homes.

It ensures that residents receive the right care, in the right place, at the right time.

🧑‍⚕️ Key Benefits for Residents

· Regular health checks and monitoring

· Faster access to GPs, nurses, and other healthcare professionals

· Personalised care plans tailored to individual needs

· Proactive management of long-term conditions

· Improved coordination between your care home, GP practice, pharmacists, and community teams

· Reduced avoidable hospital admissions

🩺 What the Service Includes

· Weekly “home rounds” with the ECP (name) and regularly supported by CC (name) and GP (name). 

· Multidisciplinary team meetings, including nurses, therapists, social care staff, and pharmacists

· Medication reviews to ensure safe and effective treatments

· Urgent care support to help manage health concerns quickly

· Enhanced end-of-life care with compassionate planning and support
